Student Activities Registration

Overview: ADM uses an online registration process for all 6-12 grade students participating in activities. All student athletes are required to submit a pre-participation physical form, concussion waiver, review and acknowledge the activities handbook, and have emergency contact information on file in order to participate in activities. Read all of the information on this page prior to starting the online activities registration process.
Online Registration: Activities Registration for the 2019-20 school year will open on July 29th. Parents and students should complete the online registration process together. A 5 digit student ID number will be needed to complete the online registration process. The student’s 5 digit number can be found on Infinite Campus.

Student Physicals: All student athletes must have a current sports qualifying physical exam on file in the Activities Office. Physicals are good for 13 months from the exact date of the last physical. If you do not have a current physical form at the time of online registration you can upload or submit a copy to the Activities Office at a later date.
- Front of Form: Must be completed by the parent.
- Back of Form: Must be completed by a doctor.
- Bottom of Form (back): Must be signed and completed by a parent.
Concussion Waiver: All student athletes and their parents are required to read and sign a concussion waiver as a part of the online activities registration process. The following fact sheet and waiver will be reviewed and digitally signed during the online registration process. Paper copies will not be accepted.
Activities Handbook: The ADM Activities handbook provides guidance and information related to parent-coach communications, activities eligibility, the activity code of conduct, and other rules. All student athletes and their parents are required to read and sign and acknowledgement of the code of conduct and the Activities Handbook as part of the online activities registration process.
